JOB SUMMARY:
Responsible for exemplifying NET Health’s (North East Texas Public Health District) Mission, Vision and Values, providing outstanding customer service. The Community Health Worker (CHW) will function as a member of a multidisciplinary team that conducts outreach to and education of men to strengthen skills and understanding of fatherhood. The CHW will promote WIC services to expectant fathers and families with young children. CHW reaches out to help them access needed resources re: personal health, employment, education, and family support and to enroll in supportive services with goals to achieve good health practices during pregnancy and early childhood. Primary responsibilities include outreach in-person and via social media, facilitation of health education and improvement, health promotion and support, and collaboration with other partners in the community to promote healthy pregnancy, birth, and parenting. Responsibilities also include teaching about early child development, including brain development. This position works as a WIC team member and reports directly to the WIC Director. Performs other related duties as required/assigned.
MAJOR TASKS AND RESPONSIBILITIES:
• Seek out opportunities to share information about WIC services in the community (at churches, barber shops, health fairs, small businesses, community gatherings, clinics, etc.).
• Be well-versed in the benefits of father involvement, infant health, maternal health, nutrition, breastfeeding and confident to present that information one-on-one, in small groups, and/or in large groups.
• Be familiar with the range of services available for pregnant women, fathers-to-be, families, infants and young children in Smith County.
• Assess for eligibility to participate in Women, Infant and Children Program.
• Assess and refer to other community services families might need, such as food, help with basic needs, healthcare, etc.
• Facilitate access of external services to ensure client needs are met.
• Design a tracking system for community referrals made and follow up on receipt of resources.
• Collaborate with WIC staff and providers to promote population health.
• Receive and follow up on referrals to assess client understanding of and willingness to enroll in WIC services.
• Work with another CHW to manage social media outreach that informs men in the community about WIC services and other resources in Smith County.
• Convene regular gatherings of men to set goals for supporting a pregnant partner and for parenting an infant or young child.
• Attend and participate in community coalitions working to serve the parenting needs of early childhood.
• Develop collaborative working relationships with other agencies and providers throughout the East Texas community to best promote engaged and responsible fatherhood.
• Work with WIC team members on special projects such as client graduation and special events for the mother, the father, and the whole family.
• Respect clients by recognizing their rights, privacy, and maintaining confidentiality.
• Handle all phone calls, emails, and in person interactions cordially and professionally.
• Conduct all interactions with cultural responsiveness and humility.
KNOWLEDGE, SKILLS, ABILITIES AND COMPETENCIES:
• Confidence in working using the Core Community Health Worker competencies.
• Knowledge of community resources and experience working with a variety of community-based organizations preferred.
• Experience in conducting community outreach.
• Motivational interviewing skills.
• Strong oral and written communication skills.
• Strong organizational and time management skills.
• Flexibility in managing a variety of types of assignments.
• Meet deadlines for assignments.
• Knowledge of men’s health and ability to educate and engage men in frank health discussions.

• Spanish-speaking preferred.
REQUIRED EDUCATION / EXPERIENCE:
• High School diploma or GED required. Two years of health/clinical experience or equivalent health-related education required.
• Current Community Health Worker certification by the Texas Department of State Health Services. Must maintain current CHW certification.
• CPR certification preferred or obtained within 3 months of hire. Must maintain current CPR certification.

May be exposed to communicable diseases.
Must be fully immunized within 90 days of employment and show proof thereof.
Must be able to spend six to eight hours walking and standing and/or sitting.
Must be able to lift up to 25 pounds and stretch to reach files.
