JOB SUMMARY: CHW CERTIFICATION REQUIRED
Responsible for exemplifying NET Health’s (Northeast Texas Public Health District) Mission, Vision, and Values, providing outstanding customer service. The Maternal Health Program Specialist will provide leadership and direction of operations, administration, evaluation of High-Risk Maternal Care Coordination Services Pilot Program. Work collaboratively with DSHS Community Health Improvement, Maternal and Child Health Unit, Healthy Texas Mothers and Babies Branch to test, assess and refine the implementation of High-Risk Maternal Care Coordination Service Pilot Program.
Major Tasks
Maternal Health Program Specialist serves as a high-level communications champion, coordinating with community partners and other departments and organizations. Develop strategies and key messages for promoting projects being done in the programs. Ability to develop material for blogs, newsletters, and social media that target multiple audiences. Assist leadership in strategic planning, providing financial oversight and accountability in collaboration with the department director and leadership.
RESPONSIBILITIES:
• Demonstrate professional management and leadership in the clinic and community practice of maternal and child health.
• Collaborate with other disciplines, departments, and support services to assure and improve patient pregnancy, birth, or postpartum outcomes.
• Oversee the implementation and testing of the HRMCCS-PP for pregnant patients at high-risk for poor pregnancy, birth, or postpartum outcomes
• Develop project plans (long and short term), setting milestones/targets, adheres to deadlines and develops recommendations for implementation for strategic initiatives.
• Serve as a point of contact for teams when multiple divisions/ departments are assigned to the same project to ensure team actions remain in synergy.
• Communicate effectively with the department director and Leadership to drive and keep projects aligned.
• Will have the ability to make effective decisions when presented with multiple options for how to progress with the project.
• Will assist with financials as directed by and in coordination with the Department Director.
• Respect clients by recognizing their rights, privacy, and maintaining confidentiality.
• Handle all phone calls, emails, and in person interactions cordially and professionally.

Duties
• Practicing intake and referrals of patients in need
• Identifying patients who are at-risk to assess eligibility for various services
• Offering and reviewing screenings or self-referrals
• Referring patients to other programs and community services
• Tracking patients to confirm program enrollment and engagement
• Providing follow-up and case management to deliver assistance and information
• Implementing patient engagement strategies to improve outcomes
• Maintaining professional boundaries and respecting patient confidentiality
• Coordinating health and wellness activities including classes and groups
• Auditing charts for quality assurance
• Documenting interactions with patients in a data record system
• Attending and participating in community or host site events
• Creating social media content to promote the programs
• Updating documentation to assess program adoption and progress
• Implementing strategies to recruit clients to attend health and wellness classes
• Conducting social determinants of health screener
• Providing social service navigation
• Maintaining, assembling, and distributing materials and supplies
• Participating in meetings and conferences
KNOWLEDGE, SKILLS, ABILITIES AND COMPETENCIES:
• Knowledge of community resources and experience working with a variety of community-based organizations preferred.

REQUIRED EDUCATION / EXPERIENCE:
• High School diploma or GED required. Two years of health/clinical experience or equivalent health-related education required.
• Current Community Health Worker certification by the Texas Department of State Health Services. Must maintain current CHW certification.
• CPR certification preferred or obtained within 3 months of hire. Must maintain current CPR certification.
• Two years minimum experience as a Program Coordinator or similar role.
• Strong working knowledge of Microsoft suite
• Solid organizational, multi-tasking, and time management skills
• Strong client-facing and teamwork skills
• Project management, business administration, or health care administration experience preferred.

May be exposed to communicable diseases.
Must be fully immunized within 90 days of employment and show proof thereof.
Must be able to spend six to eight hours walking and standing and/or sitting.
Must be able to lift up to 25 pounds and stretch to reach files.
