JOB SUMMARY: MUST HAVE EXPIENCE IN MATERNAL AND CHILD HEALTH. BILINGUAL PREFERRED.
Responsible for exemplifying NET Health’s (Northeast Texas Public Health District) Mission, Vision, and Values, providing outstanding customer service. The Community Health Worker (CHW) will function as a member of a multidisciplinary team that provides education, information, outreach, and referrals for women who are at a higher risk for poor pregnancy, birth, or postpartum outcomes within the community.
MAJOR TASKS AND RESPONSIBILITIES:
• Work with DSHS, Healthy Texas Mothers and Babies and High-Risk Care Coordination team members on special projects such as reviewing Maternal Health CHW curriculum, testing modules of CHW
curriculum, providing feedback, reports, daily documentation and assisting with special events for the mother, infant, child, and whole family.
• Engage partners within the community to identify and reduce barriers to pregnant and postpartum women’s access to health care in Smith County.
• Conduct all interactions with cultural responsiveness and humility.
• Respect clients by recognizing their rights, privacy, and maintaining confidentiality.
• Recruit and enroll pregnant and postpartum women into program according to DSHS guidelines.
• Assess clients’ program eligibility and ensure that support is addressing key needs.
• Utilize job aides and referral system that includes screening women for increased risk for poor maternal health outcomes.
• Develop client-centered care plans that foster and promote self-advocacy for their health.
• Provide short-term case work and case management services.
• Monitor and document client needs, progress, and success.
• Complete weekly, monthly, and quarterly reports as required by DSHS.
• Develop and /or maintain an organized data system of client interactions, risk assessment tools, care coordination models, data collection tools.
• Participate in staff training that focuses on building capacity and key aspects for addressing high-risk maternal needs and care coordination delivery in community/clinical settings.
• Attend other necessary trainings and meeting related to job requirements and expectations.

• Knowledge of community resources and experience working with a variety of community-based organizations preferred.
• Knowledge of maternal health and ability to facilitate monthly education classes on maternal health risk factors.
• Experience in conducting community outreach and recruitment.

• Handle all phone calls, emails, and in person interactions cordially and professionally
• Flexibility in managing a variety of types of assignments.

• Working knowledge of Microsoft Office products.
• Spanish-speaking preferred.
REQUIRED EDUCATION / EXPERIENCE:
• High School diploma or GED required. Two years of health/clinical experience or equivalent health-related education required.
• Current Community Health Worker certification by the Texas Department of State Health Services. Must maintain current CHW certification.
• CPR certification preferred or obtained within 3 months of hire. Must maintain current CPR certification.

May be exposed to communicable diseases.
Must be fully immunized within 90 days of employment and show proof thereof.
Must be able to spend six to eight hours walking and standing and/or sitting.
Must be able to lift up to 25 pounds and stretch to reach files.
