The CommUNITY Cares program was created to
connect individuals to the correct medical home.
About the CommUNITY Cares Program
This community based program is led by the
Northeast Texas Public Health District (NET Health) and consists of medical and social service partners.
All partners are dedicated to meeting the needs of the individuals referred into the program.

There is no cost to enroll in the program.
You pay only what is required by the clinic to where your appointment has been scheduled.
How to Get Started
Enroll at any of our partner locations listed to the right side of this page, so that you can sign a consent form to participate in the program.
Each clinic partner offers affordable options based on your household income.
The Enrollment Process
Your referral will be sent to our Center for Healthy Living and a community health worker will contact you to learn more about your health needs.
Your referral will be made to the best agency to meet your health needs and you will be contacted
with an appointment date/time by those agencies.
