The Center for Healthy Living is located at 2815 Frankston Highway in Tyler (map).

No appointment is necessary. Visit us and let us know how we can help you become healthier.

Parking is accessible when you turn onto Walton Road and then into our parking lot.

Free consultations are available for you to speak with one of our Community Health Workers, who are prepared to help you address your health needs.

Regardless of where you live in East Texas, regardless of how much money you earn, whether or not your have health insurance, and whether or not you already see a doctor on a regular basis, we are here to help you.  The Center for Healthy Living provides all of the below free services:

• Blood Glucose Screening
• Cholesterol  Screening
• BMI Assessment
• Help With Accessing Local Health Services
• Referrals to Local Community Resources
• Referrals to a Primary Care Physician
• Referrals to a Mental Health Specialist
• Breast & Cervical Cancer screening services
• Individualized Health Education
• Nutrition counseling

We are open Monday - Friday, from 8:00 a.m. - 12:00 p.m., and from 1:30 p.m. - 5:00 p.m.
